At your new patient exam, we will be listening. Whatever they are, we want to achieve YOUR goals first. What can we do to make you look good, feel good and be healthier?
If you are over age l6 and in our office for a dental check-up, the ADA recommends a full mouth series of diagnostic Xrays (FMX) every 3-5 years. Annually, they recommend the shortened version of 4-6 films at your visit. If you have had x-rays recently, we encourage you to have your x-rays forwarded to our office. X-rays are necessary to diagnose any decay, disease, infection and abnormalities that are not visually evident. We map your periodontal (gum) health in detail and complete a TMJ and oral cancer exam (even non-smokers). This usually takes 25 minutes to 50 minutes, but we schedule an hour, in case you have extra questions for us! If you require extensive treatment or would benefit from a review by a specialist, the doctor will create a comprehensive analysis of your existing condition. A one-on-one consultation with the doctor and financial coordinator will be scheduled to address various treatment options and any questions you may have.
Once you have become established as a patient, the doctor will determine the type of cleaning that is necessary. You will soon be scheduled with one of our experienced hygienists. If you are diagnosed with having periodontal disease and require a 'deep cleaning', additional visits may be necessary.
Complete the Health History and Office Policy form if you've received it in the mail. Please review these forms carefully. You may also download a .pdf copy (at the bottom of the page). If you do not have time to complete the forms at home, we ask that you arrive 10-15 minutes prior to your appointment time.

Please, provide us with your insurance and identifying information in advance (48 hours, business day notice is suggested). As a courtesy to our prospective patients, we volunteer to verify insurance benefits. This helps to give the patient an idea what the policy covers before incurring any charges. Dental plans usually put 'frequency limitations' on routine procedures. When we check benefits, we are also verifying any recent history of x-rays, exams and cleanings which may result in a higher out of pocket expense for the patient. Sometimes this can be as simple as a quick phone call. Other times, it can be a process of fax transmittals and online requests. So, we find that 48 hours is usually enough time to verify and research your benefit plan.

If you wish to have your previous dentist forward your dental records to our office, they may require written authorization from you. They will require your permission to share protected health information. A standard records release form is provided (below).

How You Will Find Our Office
We are located in the Hilltop Professional Building at 1855 San Miguel Dr., Suite 27, just above downtown Walnut Creek, overlooking Broadway Plaza. Our office is on the second floor, in the South West corner of the complex.
When entering our parking lot, the second floor of our two story building will be visible to you. You may park on this level for handicap access. As you approach either entrance of the 'L-shaped' building, you will find us located in the far left corner of the complex.
Lower level parking is available behind the building, with stair access to the second floor. Patients who do not require handicap access to our office usually find that the lower level parking level is within closer proximity to our suite (provided the use of stairs).
